30-year-old male with alcohol use disorder who initially presented with low sodium levels. The patient developed altered mental status following administration of hypertonic saline.

Findings & Impressions
Findings:
There is mild prominence of the sulci and cisterns. There is increased restriction along the central bilateral pons. There is corresponding hyperintensity on the T2/FLAIR sequence that spares the peripheral pons and some of the corticospinal tracts.
Impression:
Findings concerning for acute central pontine myelinolysis.
Recommendations:
For your training purposes, recommendations are not included in this report.
Acuity
Urgent: non-routine communication within 4 hours
The ordering clinician should be notified of these findings.
