Findings & Impressions
Findings:
There is a 6.2 x 5.2 cm hypodense lesion in the right thalamus with a hyperdense center. The lesion causes a 6.5 mm right to left midline shift. There is surrounding vasogenic edema extending into the right cerebral peduncle, midbrain, and pons. There is mild ventricular enlargement secondary to early obstructive hydrocephalus. Postcontrast images reveal a 2.5 cm ring-enhancing lesion in the right thalamus.
There is mild mucoperiosteal thickening is present in the right sphenoid sinus, inferior right maxillary sinus, and bilateral posterior ethmoid air cells.
Impression:
There is a 2.6 cm ring-enhancing lesion in the right thalamus with surrounding vasogenic edema and 6.5 mm midline shift, causing early obstructive hydrocephalus. Differential considerations include toxoplasmosis and CNS lymphoma.
