Findings & Impressions
Findings:
Moderate right hemothorax with multiple right-sided rib fractures (1st, 2nd, 4th-9th). There is also consolidation in the posterior right lung likely secondary to compressive atelectasis. However, this may also represent contusion and/or aspiration. There are several cystic lesions in the right lung likely reflecting simple pneumatoceles. There is a small left pleural effusion with minimal dependent atelectasis in the left posterior lung.
There is mediastinal bleeding within the posterior mid portion of the mediastinum likely secondary to venous bleeding from the T3-T4 distraction injury. There is no aortic injury. There is atherosclerotic coronary artery disease.
The NG tube is looped in the distal esophagus. There are left parapelvic renal cysts.
There is widening of the T3-T4 disc space consistent with two column injury. There is also a vertebral body fracture at T8. There are advanced degenerative changes with several anterior bridging osteophytes throughout the spine.
Impression:
Moderate right hemothorax with multiple rib fractures.
Mediastinal hematoma secondary to T3-T4 distraction and likely ligamentous injury. There is a T8 vertebral body fracture.
The NGT tube is coiled in the distal esophagus.
