Findings & Impressions
Findings:
There is a large left-sided retroperitoneal mass emanating from the mid to upper pole of the left kidney with the claw sign present. The mass measures 16.0x10.3x11.1 cm in maximum dimension and creates mass effect upon the left renal collecting system. There is a delayed left-sided nephrogram. Additionally, the left kidney is anterior, medially, and inferiorly displaced secondary to this large renal mass. A portion of this mass contains fat.
The renal vasculature is poorly seen. There may be tumor thrombus within the renal vein, but this may also be due to volume averaging. There is a large vessel supplying the left kidney, likely reflecting the renal vasculature.
There is retroperitoneal adenopathy with lymph nodes measuring up to 6 and 9 mm in size. There are scattered periaortic lymph nodes. The abdominal aorta and its branches are patent but displaced laterally due to the mass. There is a fat-containing umbilical hernia.
Impression:
Large heterogeneous retroperitoneal mass measuring up to 16.0 x 10.3 x 11.1 cm in size and emanating from the left kidney. There is associated reactive retroperitoneal lymphadenopathy. This most likely represents a Wilm’s tumor, but may also represent a clear cell sarcoma given the mass effect upon the collecting system. No signs of metastases.
