Hyperacute Seizure 13 - Clinical Case Summary
SummaryHistory
32 yo male presenting with an episode of "eye fluttering", tremors, and unreponsiveness. There is a history of a "seizure" event at age 6 years.
Exams Performed
T1-w MR precontrast only; MR Coronal T2-w spin echo and MR FLAIR; MR diffusion
Prior available imaging reports
CT Head was not obtained.
MR T1-w exam
1. The noncontrast T1 weighted exam was within normal limits.
T2-w MR
1. Subtle features of acute post seizure hippocampal injury involving the left hippoampus, as described above.
MR diffusion (DWI)
1. Negative diffusion imaging of the hippocampus and the remainder of the cerebrum.
MR Susceptibility (SWI) is not available

Overall impression
1. There is evidence of acute post seizure changes in the left hippocampus. Changes include cytogenic edema and effacement of the internal and external white matter tracts. The findings are clearly abnormal but require the coronal T2-w spin-echo imaging is viewed with correct high contrast window widths. All other sequences were unrevealing.
