Findings

Sagittal MR

There is upward bowing of the lamina terminalis and distension of the optic and infundibular recesses of the 3rd vent indicating elevated intraventricular CSF pressure.

There is downward displacement of the tuber cinereum/mammillary bodies initially reaching then passing the plane of the incisura consistent with central incisural downward (transtentorial herniation).

There is herniation of the suprasellar cistern into the intrasellar space.

There is posterior fossa mass effect produced by dilatation of the 4th ventricle, which has produced upward herniation.

There is posterior fossa mass effect produced by dilatation of the 4th ventricle, which has produced downward tonsillar herniation.

There is unexplained downward incisusal and tonsillar herniation with or without ventriculomegaly, which is most consistent with intracranial hypotension (ICH).

Other significant imaging findings are present.
