66 yo male who fell and has significant depressed level of consciousness at sceneExam

Prior Study
Bone window CT head1. Right sided, nearly horizontal, linear, fracture extending through the squamous part of the temporal bone into the parietal bone.
2. This fracture appears to cross the middle meningeal artery groove and has free air adjacent to its inner and outer margin consistent with a penetrating injury in the absence of any other source.
3. The presence of intra and extracranial free air adds infection risk to the clinical course.
