History

28 yo female presenting acutely with new onset headache and a seizure. But the prior history was unknown at the time of this admission. Later it was discovered the patient was diagnosed 5 years earlier with a left parietal AVM, which had been treated with intravascular embolotherapy and radiation.

Findings

Current vascular findings on CTA/MRA/DSA:

There is evidence of pial avm.

There is evidence of deep parenchymal avm.

There is evidence of choroidal avm.

There is an AVM is present. Therefore assess: the afferent input; nidus detail, efferent output; internal AVF; steal effects; venous hypertension effects.

There is evidence of angiomatous transformation (AVM without distinct nidus)

There is evidence of thrombosed avm.

There is evidence of proximal saccular, flow-related aneurysm(s) evident in afferent arteries to an AVM.

There is evidence of AV malformation with a high-flow AV-fistulous component.

There is a direct AV pial fistula (mostly in infants).

There is evidence of Galenic AV malformation or intradural direct AV high flow fistula (predominantly infants & children).

There is evidence of dural avm on CTA/MRA/DSA.

There is evidence of dural avm with efferent drainage into pial veins.

There is evidence of dural AVF or AVM with concurrent sinus thrombosis & rerouted venous egress.

There is evidence of non-shunting, cavernous malformation(s).

There is evidence of a DVA.

There is evidence of pontine telangiectasia.

CT perfusion is abnormal for any reason.
