CB1557-Spinal lymphoma, MR

History

25-year-old (M) patient with Hodgkins lymphoma with bilateral lower extremity weakness, hyperreflexia and possible urinary retention.

Exam

Cervical and Thoracic MRI - T1, T2 and STIR sequences were variably performed in the axial and sagittal plane with high-resolution techniques focused on the most suspicious levels based on clinical examination. These include images before and following the intravenous administration of a paramagnetic contrast agent.

Findings

Paraspinal soft tissues

There is a mass, swelling, or abnormal signal change present in the prevertebral or anterolateral paravertebral soft tissues. There is a mass, swelling, or abnormal signal change present in the posterolateral paravertebral or posterior paravertebral soft tissues.

Spine and epidural space

There are destructive/erosive process, mass, or abnormal signal changes/contrast enhancement present the vertebral body, the vertebral endplates, and posterior vertebral elements at T3.

There is compression with retropulsion into the spinal canal at T3. There is abnormal alignment.

There is an epidural disease process present causing compression of the thecal sac and related neural elements.

Thecal sac and subarachnoid space

The thecal sac is extrinsically abnormal.

Spinal cord

The spinal cord is displaced and compressed. There is a spinal cord abnormality that alters its signal intensity.

Impression

Pathologic T3 compression fracture with retropulsion, canal narrowing, and spinal cord compression with edema are present. There is prevertebral and circumferential epidural enhancing soft tissue with mass effect on the dural sac. Abnormal marrow infiltration involves the T7 vertebral body and posterior elements without compression. Diffuse osseous decreased T1 and T2 signal present in the spine. Radiological findings are consistent with lymphoma with pathological compression fracture and spinal cord compression.
