History
13 yo male presenting with 1 week of bifrontal headache and vomiting without systemic symptoms. Imaging revealed a central intraventricular supratentorial mass; the histology of the mass is unknown at the time of the imaging.Exam
MR flair
Prior Study
MR T1-w pre or post contrast MR1. There is a 3 cm enhancing, well marginated mass, apparently originating from the septum pellucidum, which presents in the midline; it is centered in the cistern of the velum interpositum. It exhibits features of a central neurocytoma. There is one drop metastasis in the body of the right lateral ventricle.
2. There is grade 1.6/4 lateral ventricular dilatation with obstruction a the foramina of Monroe
3. There is a sequestered, dilated 3rd ventricle with downward transtentorial displacent of the tuber cinerum to just below the plane of the incisura.
Sagittal MR
1. There is a 3 cm enhancing but well marginated mass is present in the midline; it is centered in the cistern of the velum interpositum at the base of the septum pellucidum with features of a central neurocytoma. There is on drop met in the body of the right lateral ventricle.
2. There is grade 1.6/4 lateral ventricular dilatation with obstruction at the foramina of Monroe
3. There is a sequestered, dilated 3rd ventricle with downward transtentorial displacent of the tuber cinerum to just below the plane of the incisura.
