The basilar artery is hyperdense, concerning for thrombotic occlusion. There is subtle loss of grey -white differentiation and hypoattenuation throughout the left greater than right cerebellar hemispheres, as well as the midbrain and pons.
There is no acute intracranial hemorrhage. There is no mass effect or midline shift. There is no hydrocephalus or herniation.
Impression:
Findings highly concerning for acute basilar artery thrombosis and developing left greater than right cerebellar and midbrain/pontine infarcts.
