Findings & Impressions
Findings:
There are prominently noncalcified atherosclerotic plaques throughout the abdominal aorta. There is approximately 30% luminal narrowing of the celiac artery from atherosclerotic disease. There is also non-occlusive scattered calcified plaque in the renal and superior mesenteric arteries. The inferior mesenteric artery is proximally occluded with distal reconstruction. There is a ruptured infrarenal abdominal aortic aneurysm with contrast extravasation into the retroperitoneum extending down the left iliopsoas muscle. The aneurysm measures 5.9 x 4.9 cm and extends to the bifurcation. There is a right common and external iliac artery stent graft with total occlusion through the common iliac portion and distal external iliac reconstitution. The left common iliac artery is completely occluded with scant contrast in the left internal and external iliac arteries. There is reconstitution in the left femoral artery. In the lower lung, there is minimal groundglass opacity within the dependent portion of the left lower lobe which may be infectious or inflammatory. There are numerous simple cysts in bilateral kidneys. There is a small right hydrocele, multilevel degenerative spine changes, and left iliac vein irregularity.
Impression:
Large infrarenal abdominal aortic aneurysm with retroperitoneal rupture.
Severe peripheral vascular disease consistent with previous history.
