Findings & Impressions
Findings:
Right upper lobe mass resulting in occlusion of the right upper lobe bronchus and right upper lobe pulmonary artery branch. The bronchial obstruction results in partial atelectasis of the right upper lobe. The mass also encases and narrows several right pulmonary artery branches. There is right perihilar and right paratracheal lymphadenopathy. There is also an enlarged lymph node in the AP window. There are satellite nodules in the right upper lobe. There is right lower lobe subsegmental atelectasis. There is a lytic vertebral mass of the T2 and T3 vertebral bodies and left sided pedicles, articular pillars, and laminae. There is also bony contiguous destruction of the posteromedial left second and third ribs.
Mixed emphysematous changes.
Multilevel thoracic spine degenerative changes.
Impression:
Large right perihilar mass with post-obstructive right upper lobe atelectasis. There are satellite nodules and right hilar and paratracheal lymphadenopathy.
There is a destructive soft tissue lesion in the upper thoracic spine and adjacent ribs. The mass may extend into the spinal canal.
These findings are concerning for metastatic endobronchial carcinoma.
